How to Register
Clay County Emergency Management, in partnership with the Florida Department of Health -Clay County maintains a registry of residents with special medical needs for two purposes: 1) to pre-register individuals who, based on their medical needs, are good candidates to utilize a Special Needs Shelter and 2) to provide officials with the location data of vulnerable populations in the event of an emergency such as an evacuation necessitated by a wildfire or chemical accident. This link allows you to fill out the registration online.
This is a PDF copy of Clay County’s Special Needs Registration Form which individuals can print out at home, fill out and mail back to the county for review. Simply follow the instructions on the form.
Special Medical Needs Shelter
Registration does not guarantee that a person will receive assistance during an event. Rather, the information within the registry helps provide important information to local emergency management officials, which will assist them in better planning for the needs of the county’s special medical needs population in an emergency.
Registered individuals will have their information evaluated to determine if they are candidates to utilize the county’s sole Special Medical Needs Shelter. This shelter is a refuge of last resort for individuals who have special medical needs and must evacuate. Sheltered individuals should be accompanied by a caregiver for duration of their stay.
